During your first visit with us, Drs. Melissa and Randall Stewart will answer any questions or concerns you may have about your dental health, conduct a thorough oral exam, and if further treatment is required, develop a customized treatment plan to fit your needs. We will also provide you with a tour of the office and take the time to get to know you a little better. We believe that dentistry is a partnership between dentists and patients, and we are excited to partner with you and help you reach your smile goals!

To save time checking in at your appointment, you may download the following forms, print them out at home, fill in the required information and bring them with you to your appointment. If you have any questions or are having trouble accessing the forms, please contact our office.
Financing and Insurance
At Stewart Dental Group, we want to help you get the most out of your dental benefits. We accept most traditional insurance plans, and are happy to verify the acceptance of your plan if you are unsure. We currently do not participate in Health Management Organizations (HMO). Dr. Melissa Stewart is a Delta Dental Premier Provider.
Payment Options
For your convenience, we offer several different forms of payment. Payment options include cash, personal checks, major credit cards and money orders. If you ever are unsure about whether a form of payment is accepted, please contact our office.
